Health Insurance Costs in Spain
What should you budget for private medical cover? The answer depends on your age, postcode and the policy you choose. Below, compare published price examples and learn how to look beyond the monthly premium.
For context, one insurer’s April 2026 tariff lists individual premiums of €64.58 a month with co-payments or €93.96 without co-payments for ages 40–44 in its “Resto” zone. These are examples, not a market average or a personal quote. See the table and conditions below.
Published Health Insurance Prices by Age
These examples come from Sanitas’s April 2026 tariff, page 3: Más Salud, individual policies, “Resto” pricing zone. Other zones and insurers have different prices. Amounts are per person.
| Age | Óptima With co-payments | Sin Copago Without co-payments |
|---|---|---|
| 20–24 | €54.54/month | €82.94/month |
| 30–34 | €58.97/month | €89.53/month |
| 40–44 | €64.58/month | €93.96/month |
| 50–54 | €83.48/month | €111.67/month |
| 60–64 | €142.56/month | €175.93/month |
| 65–69 | €164.81/month | €201.31/month |
Conditions: the published premiums apply to the first policy year for new customers starting between 1 April and 31 December 2026. An additional Consorcio surcharge of 0.15% applies. Prices can vary by residence. Eligibility, underwriting and policy conditions still apply. Checked 10 October 2026; ask for a current quote before buying.
Why the Cheapest Advertised Price May Not Be Your Price
A starting price is tied to a particular customer profile. For example, DKV’s Integral page advertises a promotional €17 monthly starting price for Complet, but its example is for a nine-year-old living in Zaragoza, Asturias or Málaga. It also has co-payments. That figure should not be used as an adult’s budget.
When you see a low headline price, ask: whose age and postcode is it based on, which benefits does it include, are there charges when using the policy, and how long does the discount last? Compare written quotes for the same people and start date.
Compare the Annual Cost, Not Just the Monthly Payment
A useful comparison adds together the premium for the whole year, expected co-payments and any services you expect to pay for yourself. Keep optional extras and payment charges visible. Ask whether a monthly payment is an instalment of an annual contract.
A Simple Budget Example
Imagine two hypothetical quotes with otherwise comparable cover. Policy A costs €60 per month plus usage charges. Policy B costs €85 per month without co-payments for covered services. Their annual premiums are €720 and €1,020: a difference of €300.
If your usage charges under Policy A came to €120, the total would be €840. If they reached €400, the total would be €1,120. This is a calculation example, not an insurer offer or a prediction of your medical needs. Actual co-payment schedules, caps and benefits must be compared.
A no-co-payment policy still has defined benefits, exclusions and limits. It does not mean every treatment or every private doctor is included. Sanitas’s co-payment guidance explains that charges depend on the service and contract.
What to Check Before Comparing Quotes
The Same Level of Cover
Ask each insurer to quote for the same needs. Does the proposal include hospital treatment and surgery, or mainly consultations and tests? If you want reimbursement outside a medical network, compare the amount you must pay yourself and the claim limits. A cheaper policy with different benefits is not a like-for-like alternative.
Your Local Doctors and Hospitals
Write down the facilities you would actually use. In Sotogrande and the surrounding area, that could mean checking a particular local clinic as well as a hospital further away. Confirm the doctor, location and service under the exact policy being offered. Ask about any referral or prior-authorisation requirements.
Acceptance, Waiting Periods and Exclusions
Request the policy wording and written acceptance terms. Check when each relevant benefit becomes available and whether any exclusions apply to you. Tell the insurer what it asks in its health questionnaire accurately. A quoted premium alone does not confirm that an existing condition will be covered. Our medical insurance guide explains these checks in more detail.
Discounts and Renewal
Ask for the first-year total and how any promotional discount changes later. Find out whether a quoted renewal illustration is guaranteed or only indicative. A discount lasting several years does not necessarily fix the underlying premium for those years. Keep the policy renewal date and cancellation requirements with your paperwork.
Budgeting for Couples, Families and Retirement
For a couple or family, ask for both individual and joint-policy quotations where available. Compare the total for everybody, including any differences in cover. A family discount is useful only if the resulting policy fits each person’s needs.
For retirement, request a quote using your current age and planned start date. Check entry-age eligibility, hospital benefits and renewal terms explicitly. Avoid using a younger adult’s price as your budget. If you already have cover, obtain written acceptance for the replacement before cancelling it.
If the Insurance Is for a Visa or Residency Application
Mention the exact application route before requesting prices. Ask the broker and insurer to check the policy and certificate against the current requirements of the authority handling your application. Do not choose on price first and assume the cheapest policy will qualify. See the residency section in our health insurance guide for the questions to raise.
Common Questions
How Much Does Private Health Insurance Cost in Spain?
There is no single price for every applicant. The dated tariff examples above show how one product’s price changes by age and co-payment option. Use them for context, then obtain a quote for your postcode, age, intended start date and required benefits.
Is Cover Without Co-payments Always Better?
It can make the cost of covered appointments more predictable, but the premium may be higher. Compare your budget, the usage-charge schedule and the actual benefits. Neither payment structure removes policy exclusions.
Can Iberica Help Me Compare Options?
Yes. Iberica Insurance can help you explore medical cover from different insurers. Start with your location, who needs insurance, when you want it to begin and whether it is for a visa or residency application. We will explain the information needed for a personal quotation and the next steps.
